Mrs. Jones, had a general muscle weakness which has been getting worse over the past five months, she has little interest in her regular daily activities and experiencing fatigue without physical exertion. Recently, she has been experiencing headaches more frequently at night over the past few weeks. In addition, she reported having irregular menstrual cycles for the past one year with unexplained weight gain in her abdomen.
She was diagnosed with D.M (type II) since 2010 and on regular treatment. She was treated for hip pain related to bursitis in 2019 and her physician prescribed multiple corticosteroid injections. Patient has a history of chronic Asthma and been taking inhaled steroids for that last 8 years. Patient was treated for hyperplasia of the adrenal cortex. Lastly, she is a hypertensive patient for the last 10 years on regular antihypertensive drugs.
Physical examination findings:
Pt presents with purple/pink stretch marks on arms, abdomen, and thighs. Pt has multiple cuts and bruises on her arms and hands with an explanation of having thin skin. Pt is obese with noticeable fatty deposits in the upper back and midsection
